Health tech startup Kouper emerges from stealth with $10M in funding to transform patient care transitions

Kouper has emerged from stealth with $10M to fix one of health care’s most overlooked failures: patient transitions.
Kouper, a new health tech startup focused on making hospital discharges safer and smoother, has officially launched with $10 million in fresh funding. The round was led by General Catalyst, with participation from 25Madison and CVS Health Ventures. The news lands amid rising investor interest in AI-driven health tech.
For decades, the handoff between hospital care and what comes next is one of the most fragile moments in a patient’s journey. It’s also where things often fall apart. Poorly coordinated discharges can lead to confusion, missed follow-ups, and worse outcomes, costing the U.S. over $53 billion annually in avoidable readmissions. Kouper is trying to stop that spiral before it starts.
Kouper Launches with $10M to Fix Health Care’s Most Broken Moment: Hospital Discharges
Founded by CEO Salman Ali (co-founder of GetSnooze) and CTO Ablimit Keskin, PhD (formerly of Komodo Health), Kouper builds software that plugs directly into hospitals and health systems. It tracks patients in real time after they’re discharged—whether from the hospital, ER, or urgent care—using AI to automate scheduling, surface red flags, and keep patients engaged. And it does all of this without adding more work for clinical teams.
With fresh funding, Kouper is scaling its team and deepening its work with early health system partners.

What Kouper is really building is the plumbing for better handoffs in health care. Its platform gives hospitals and care teams a more complete, real-time view of each patient after discharge. Instead of fragmented spreadsheets or disconnected systems, Kouper brings everything into one place so decisions can be made quickly, based on accurate context.
And it doesn’t require big teams or heavy implementation. Hospitals can get started with Kouper in under a month. According to the company, some partners have already seen patient engagement jump by more than 50%, and follow-up appointment rates improve by over 60%.
